You are entitled to health care that is reimbursed out of the basic health care package of the Dutch Health Insurance Act (Zvw) or the Act Long-term Care (Wlz).

Before you go to the Netherlands, you need an international health insurance certificate. Ask for this certificate at your current health insurance company. Do you already have a certificate? Please make sure to check if it’s still valid.
Tip! Make a copy of your international insurance certificate. Your caregiver needs this copy.
There are several international health insurance certificates
- European Health Insurance Card (EHIC)
With the EHIC you will receive medical care if needed during your stay in the Netherlands. - Temporary certificate instead of the EHIC
Have you lost the EHIC? Or is the EHIC stolen? Ask your current health insurance company for a temporary certificate. - Form S2 (previously E112)
Are you going to the Netherlands for a specific treatment? Before you leave, ask for the S2-form at your health insurer. Via the form they can give you permission for the treatment.
The S2-form is proof to show that you have permission for your planned treatment, if you happen to stay in:- the European Union
- Iceland
- Liechtenstein
- Norway
- Switzerland
- Form DA1 (previously E123)
Do you have an illness or complaint regarding your job? Or did you have an accident at work? You need this form to get a reimbursement.
How do I submit a claim?
Please use the online claim form to submit your bills. After submitting your bill you will receive a confirmation e-mail. You will then receive further correspondence by regular mail. You can submit the original bill up to 3 years following the treatment date or received health care.

In those cases we do not reimburse the costs of your care. That is because there are no existing agreements about this matter between the Netherlands and the country where you are insured.
How do I submit a claim?
Please use the online claim form to submit your bills. After submitting your bill you will receive a confirmation e-mail. You will then receive further correspondence by regular mail. You can submit the original bill up to 3 years following the treatment date or received health care.
