Does my care provider have a contract with Zilveren Kruis?
In the Zorgzoeker (care provider search tool) you will find the care providers with whom we have a contract. Further information in Dutch only.
How do I change or stop my collective health insurance?
You can do that in Mijn Zilveren Kruis. Log in safely with your DigiD. The start date of the change is the 1st of the month after your right to the collective has stopped or earlier if you prefer.
I'm calling for someone else. What do I need?
You need permission from that person. Without it, we cannot discuss personal details with anyone else due to privacy. The person who gives your permission can complete the machtigingsformulier. Further information in Dutch only.
